2019 Pediatric Grand Rounds - Transitioning Adolescents to Adult Care: Are YOU Prepared?

This presentation will inform participants about why it is important to provide youth with transition services and supports and how well the pediatric providers in Florida are currently addressing their patients need for such services.  The benefits of a structured approach will be discussed, and the Six Core Elements Approach to Transition will be presented.  A detailed description of each element will be provided, along with a toolbox of state and national resources which practitioners can use as they take steps to integrate transition processes and procedures into standard office practice.
